Home Health Services |

This includes health care and related services provided to people in their homes or places of residence, and is often an alternative to hospital or nursing home care. It is designed to help people get and stay well, and remain independent at home. Home care is for any medical problem that can be appropriately managed at home, including recovery from illness or surgery, chronic disease care, physical disabilities and terminal illness. Community Alternatives Program (CAP)910 872-6257
CAP is a special Medicaid program that provides in-home services as an alternative to nursing facility or institutional placement. Our program targets disabled adults (CAP/DA) and children (CAP/C) who would be forced to choose nursing home or institutional placement over home care due to their medical condition.
Services offered:
In-Home Aide Services – Level II and Level III Personal Care
Respite Care
Waiver supplies – incontinent supplies, oral nutritional supplements, medical dispensing boxes
Case Management
Telephone Alert Services
Home Mobility Aids
A CAP client may also receive regular Medicaid services under the guidelines for those services.

Personal Care Services
Private Duty Nursing
The total cost of all Medicaid provided services in the home must be maintained within the cost limit set by Medicaid. Support Staff
